The Delaware Division of Public Health (DPH) is sending this health advisory to inform medical professionals that DPH has established a standardized process for requesting, distributing, documenting, and returning medications and vaccines for approved public health activities.\

This advisory provides a synopsis of the Medication & Vaccine Request Form, and the Medication/Vaccine Delivery & Transfer Form, and explains how these documents function together to support public health response operations.

Background
DPH maintains a Medication & Vaccine Access Program to ensure timely availability of critical medical countermeasures during outbreaks, emergencies, shortages, and other approved public health activities. The program outlines how providers request products, how DPH authorizes and distributes them, and how inventory and documentation must be maintained.
The attached documents define the operational workflow:
• The Request Form initiates provider requests.
• The Delivery/Transfer Form documents distribution to external partners.
These documents support activities such as disease outbreak response, prophylaxis, community vaccination clinics, mobile health operations, school vaccination events, long‑term care response, and other DPH‑approved uses.

Recommendations
Health care providers and facilities may utilize the Medication & Vaccine Access Program for any approved public health activity requiring timely access to medications or vaccines. Authorized prescribers (MD, DO, APRN, PA, DDS, DMD, PharmD) may submit requests using the Medication & Vaccine Request Form.

DPH will review each submission to ensure appropriateness, alignment with public health needs, and availability of inventory. Requests may be approved, modified, or denied based on these criteria. This flexible approach allows the program to be used across multiple scenarios, including outbreaks, shortages, and other emergent or routine public health activities.

Authorized prescribers participating in the program must:
• Complete the Medication & Vaccine Request Form with sufficient detail for review and return to DPH by emailing to Medaccess@delaware.gov.
• Maintain proper storage and cold‑chain requirements for all products received.
• Document administrations in the patient medical record.
• Enter all administered vaccines into DelVAX within 72 hours.
• Retain required inventory and administration records.
• Return unused vaccine using the DPH Vaccine Return Form. The form is available upon request from Immunizedph@delaware.gov.
• Comply with all applicable federal, state, and local requirements.

DPH will:
• Review requests for eligibility and appropriateness.
• Verify inventory availability.
• Coordinate delivery or pickup using the Medication/Vaccine Delivery & Transfer Form.
• Maintain accountability and conduct audits as needed.
• Deny or modify requests that exceed reasonable quantities or fall outside approved public health purposes.
This review process ensures the program remains adaptable and applicable to a wide range of public health situations without requiring additional prescriber documentation.
